Conduct comprehensive assessments of patients with complex medical needs, including pain, symptom burden, psychosocial concerns, and spiritual care needs.
Develop, implement, and adjust individualized care plans in collaboration with attending physicians, nurses, social workers, and chaplains.
Provide expert symptom management for issues such as pain, dyspnea, nausea, anxiety, and delirium.
Facilitate advance care planning discussions and assist patients and families with understanding goals of care and treatment options.
Participate in family meetings to ensure alignment between medical recommendations and patient preferences.
Provide palliative care consultations for hospitalized patients and assist with transitions of care to home, hospice, or other settings.
Educate patients, families, and healthcare team members on palliative care principles.
Contribute to quality improvement initiatives and participate in palliative care education for trainees.
Qualifications:
Current NYS licensure as a Nurse Practitioner or Physician Assistant (or eligibility)
National board certification as an NP or PA
Experience in palliative care, oncology, geriatrics, or critical care preferred
Strong communication skills, empathy, and ability to work collaboratively within a multidisciplinary team
Commitment to patient- and family-centered care
Competitive base salary range: $126,000-$147,000 commensurate with experience.
