Description

Shape the Education That Shapes Patient Care

At Transitions Hospice, we believe that exceptional end-of-life care begins with exceptional education. Every clinical skill sharpened, every competency verified, every gap closed — it traces back to the educator who made it happen.

We've built a hospice organization across four states grounded in compassion, quality, and accountability. Now we're looking for a strategic nurse-leader to own and elevate our entire education infrastructure — connecting what our teams learn directly to how our patients are cared for.

If you're an RN who wants your leadership to leave a lasting mark on a care organization — and on the patients it serves — this is your role.

What You'll Do

As Director of Education, you'll be the organization's education anchor — designing programs, leading state-specific educators, and building the systems that keep our clinical teams sharp, compliant, and confident. Your day-to-day includes:

  • Leading and supervising state-specific educators across IL, IN, MI, and OH
  • Administering the Relias LMS platform, maintaining =95% compliance org-wide
  • Translating QAPI findings, audit scores, and CAHPS results into documented, measurable education interventions
  • Building competency tracking and education dashboards for senior leadership
  • Overseeing new employee orientation, clinical competency development, and ongoing regulatory training
  • Planning and managing the annual education calendar, budget, and multi-site events
  • Ensuring all clinical staff meet federal, state, and accreditation training requirements before patient assignment
  • Presenting monthly education summaries to the CNO and senior leadership team
  • Stay current on CMS regulatory updates, CHAP standards, state-specific licensure requirements, and adult education best practices; proactively update programs to reflect changes

Benefits We Offer

  • Competitive Pay
  • Medical, Dental & Vision Insurance
  • Life Insurance
  • 401(k) with company match
  • Paid Time Off
  • Company provided equipment
  • Travel Reimbursement for multi-site responsibilities
Requirements

What You Bring

  • Active, unrestricted RN license in IL, IN, MI, or OH, willingness and ability to obtain additional state licenses as needed
  • BSN required; Master's in Nursing, Education, or Healthcare Administration preferred
  • 5+ years of hospice experience, including 2+ years in a leadership, education, or quality role
  • Experience designing and delivering adult education programs in a healthcare setting
  • Working knowledge of CMS Hospice Conditions of Participation and CHAP or equivalent accreditation standards
  • Proficiency with LMS platforms — Relias experience strongly preferred
  • Experience with HCHB clinical documentation system
  • Familiarity with QAPI methodology and performance improvement processes
  • Experience supervising educators or clinical staff in a field-based model
  • Willingness to travel regularly across all license locations

Bonus points for CHPN or CHPCA certification, or experience in a multi-site hospice organization.
