Full Time Home Hospice Registered Nurse (RN)

We are hiring a Full-Time Registered Nurse (RN) for a Hospice Case Manager role, providing compassionate care to adult and geriatric patients in their homes.

What You’ll Do:

• Provide in-home hospice nursing care for adult and geriatric patients
• Coordinate and manage individualized care plans
• Complete timely and accurate documentation (training and support provided)
• Travel within your assigned territory for patient visits
• Collaborate with the hospice care team and patient families

Requirements:

• Active RN license in the state
• Must be a U.S. Citizen or Green Card holder
• At least 1 year of adult or geriatric med-surg experience
