Position Summary
We are seeking a compassionate and experienced Hospice Admissions RN to join our growing team. This role is essential in initiating hospice care during weekend hours, ensuring patients and families receive timely, empathetic support.
Responsibilities
- Conduct hospice admission assessments in homes, facilities, or hospitals.
- Evaluate patient eligibility based on Medicare guidelines and Sequoia policies.
- Educate patients and families on hospice philosophy and services.
- Complete informed consent and admission documentation.
- Initiate care plans and communicate with the interdisciplinary team.
- Collaborate with physicians, case managers, and referral sources.
- Provide emotional support during the admission process.
- Maintain accurate documentation in the EMR system.
- Respond to urgent weekend admissions and support triage as needed.
Qualifications
- Active RN license in California.
- Minimum 2 years of nursing experience (hospice, palliative care, or home health preferred).
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently during weekend hours.
- Valid driver’s license and reliable transportation.
