  • Actively grow hospice admissions through strategic business development efforts
  • Position Choice as the preferred hospice provider in the market
  • Educate referral sources on hospice eligibility, services, and benefits
  • Collaborate with clinical teams to ensure seamless patient transitions
  • Execute targeted outreach and community engagement strategies
  • Consistently meet or exceed monthly admission goals

What We’re Looking For

  • Required: Proven experience in hospice or home health business development/liaison role
  • Demonstrated success in meeting or exceeding admission/census goals
  • Established relationships within the local healthcare community (strongly preferred)
  • Deep understanding of hospice services, regulations, and referral processes
  • Highly self-directed, with the ability to manage a territory independently
  • Valid driver’s license and auto insurance
  • HomeCare HomeBase experience (preferred)

Benefits & Perks

  • Medical, Dental, and Vision Insurance
  • 401(k) with immediate vesting (100% match on first 3%)
  • HSA, Life Insurance, Short/Long-Term Disability
  • PTO & Paid Holidays
  • Company vehicle (after 90 days, if eligible)

Why This Role Stands Out

This role is designed for a seasoned hospice liaison who wants the autonomy to grow a territory and make a direct impact on patients and families through increased access to end-of-life care.
