Presbyterian Homes & Services – Optage Hospice is seeking an experienced RN or LICSW looking to use their knowledge and skills in a leadership position as a Hospice Clinical Supervisor.

This full time position operates Monday-Friday, daytime office hours, participates in supervisor on-call responsibilities with some flexibility to work remotely.

The Hospice Clinical Supervisor will serve as a leader providing supervision for a team of professional registered nurses. This position has responsibility as a member of the hospice management team to participate in the planning and implementation of strategies and programming that ensure high quality client care while achieving short and long-term operational goals and objectives for the Hospice program.
In conjunction with the essential functions of the clinical supervisor, this addendum displays the areas of focus and responsibilities for the oversight of the HOSPICE ADMISSIONS & VISIT NURSE TEAMS.
1. Referral, Intake, and Admissions Oversight
• Provide primary oversight of referral intake and admissions coordination to ensure timely, accurate, and complete referral workups.
• Balance intake workloads, ensuring appropriate staffing coverage and support.
• Ensure documentation is completed accurately and within required timeframes.
• Facilitate timely, clear communication across all stakeholders throughout the admissions process.
2. Visit Nurse & Admissions Nurse Team Management
• Ensure nurses are cross‑trained to support both visit and admissions responsibilities.
3. Transfers, Re‑Admissions, and Face‑to‑Face Oversight
• Oversee hospice transfers and re‑admissions, ensuring all required records are obtained, reviewed, and accurately entered into HCHB.
• Coordinate and secure nurse practitioner or physician support for required face‑to‑face visits related to re‑admissions.
• Communicate re‑admission plans clearly to admissions staff and service coordinators.
• Identify and deploy appropriate clinical resources for in‑person assessments as needed.
• Ensure coroner pre‑registration processes are completed in accordance with policy when applicable.
