We are looking to bring on a Hospice Clinical Manager Assistant (LPN) to support the coordination and day-to-day clinical operations of hospice care provided in patients’ homes. This role works closely with the Clinical Manager to help ensure high-quality patient care, regulatory compliance, and smooth clinical operations. The Clinical Manager Assistant will support the clinical team, assist with documentation and audits, and help maintain communication across the interdisciplinary team, patients, and families.

Support Clinical Operations
Assist with coordinating patient care, supporting field staff, and helping with chart audits, documentation review, and QAPI activities.
Support the Team
Help onboard and orient new staff, provide day-to-day support, and reinforce policies and best practices.
Ensure Compliance
Assist with maintaining accurate clinical records and ensuring compliance with Medicare, State, and ACHC guidelines.
Patient Care
Provide direct patient care in the home as needed within LPN scope.
Meetings and Coordination
Participate in IDG meetings, audits, and team discussions. Help ensure clinicians have what they need to be successful.
Professional Growth
Stay up to date through ongoing training and development.
Qualifications:
Current Virginia State LPN License
At least one year of clinical experience in hospice, home health, or a related setting
Previous experience supporting clinical operations or leadership is a plus
