DescriptionOverview:
The Hospice LVN Clinical Coordinator is responsible for coordinating clinical services and supporting the interdisciplinary team in delivering quality hospice care. This role involves supervising LVNs, ensuring compliance with care plans, and facilitating communication between staff, patients, and families. The Clinical Coordinator plays a key role in maintaining documentation standards and supporting the overall clinical operations of the hospice program.
Essential Functions:
- Coordinate clinical services and supervise LVNs in the hospice program.
- Ensure compliance with individualized care plans and regulatory standards.
- Support interdisciplinary team meetings and facilitate communication among staff.
- Monitor patient care documentation for accuracy and completeness.
- Assist with scheduling and staffing to ensure adequate coverage.
- Provide education and support to clinical staff regarding hospice protocols.
- Participate in quality assurance and performance improvement activities.
- Serve as a liaison between clinical staff and administrative leadership.
QualificationsJob Qualifications:
Education:
Licensed Vocational Nurse (LVN) with current state licensure. Additional certification in hospice or palliative care preferred.
Experience:
Minimum of 2 years of experience in hospice or home health care, with at least 1 year in a supervisory or coordinator role.
Skills:
- Strong clinical knowledge of hospice care practices.
- Excellent communication and organizational skills.
- Proficiency in EMR systems and documentation standards.
- Ability to lead and support a team of clinical staff.
- Knowledge of state and federal hospice regulations.
Environmental and Working Conditions:
- Works in office and patient care environments.
- May require local travel to patient homes and facilities.
