As a Hospice Case Manager (RN), you will coordinate and deliver individualized, patient-centered care in homes and care facilities. You’ll build meaningful relationships with patients and families while working both independently and collaboratively with an interdisciplinary team.
In this role, you will play a vital part in supporting comfort, dignity, and quality of life, while guiding families through the caregiving journey with compassion and expertise.
Key Responsibilities
- Develop, implement, and manage individualized plans of care that reflect each patient’s goals and needs
- Deliver comprehensive, patient-centered nursing care in home and facility settings
- Coordinate medical, emotional, and spiritual support services with the interdisciplinary team
- Perform ongoing clinical assessments and adapt care plans as conditions evolve
- Administer medications and manage complex symptoms to promote comfort and safety
- Educate and support patients and families on disease progression, caregiving, and symptom management
- Serve as a trusted advocate, helping patients and families make informed end-of-life decisions
- Communicate effectively with physicians and team members to ensure seamless, high-quality care
- Provide emotional and psychosocial support throughout the end-of-life journey
- Maintain accurate and timely documentation in the EMR
- Participate in interdisciplinary team meetings and contribute to quality improvement efforts
- Practice at the top of your license with autonomy, compassion, and confidence
