Hospice Overnight On-Call Registered Nurse (RN)

We are seeking a Full-Time On-Call Registered Nurse (RN) for a hospice and home health position covering the Kansas City, Missouri and Kansas City, Kansas area. This role provides compassionate, skilled nursing care to hospice patients while supporting families during end-of-life care.

Schedule

• Full-Time On Call

• Monday–Thursday | 5:00 PM – 9:00 AM

• Includes on-call responsibilities per agency policy

Role Overview:

Under the direction of the Director of Clinical Services and Hospice Medical Director, the On-Call RN supports hospice patients and families by delivering high-quality, compliant, comfort-focused nursing care while coordinating with the interdisciplinary team.

Key Responsibilities

• Provide skilled hospice nursing care and on-call support

• Perform initial and ongoing comprehensive patient assessments

• Coordinate care with physicians, IDG members, aides, and contracted services

• Monitor patient conditions and communicate changes promptly

• Develop, update, and maintain individualized plans of care

• Provide education to patients and families regarding the dying process and care decisions

• Complete hospice evaluations, informational visits, and death visits per policy

• Supervise Hospice Aides (minimum every 14 days) and LPNs (minimum every 30 days)

• Document all clinical notes accurately and timely using electronic charting

• Participate in IDG meetings, QAPI activities, and case conferences

• Ensure compliance with Medicare, Medicaid, state, and accreditation standards

• Assist with discharge planning and continuity of care as needed

Requirements

• Active Registered Nurse (RN) license in Missouri

• Willing to obtain a Kansas RN license if not already held

• At least 1 year of clinical experience (hospice strongly preferred)

• Strong assessment, organization, and communication skills

• Valid driver’s license with reliable, insured transportation

• U.S. Citizen or Green Card holder (no exceptions)
