We are hiring a Full-Time Registered Nurse (RN) for a Patient Care Manager (PCM) position in the Hospice Home Care setting.
This is a leadership opportunity for an experienced RN who enjoys mentoring staff, supporting quality patient care, and helping lead a compassionate hospice team.
What You’ll Do:
- Oversee delivery of patient services and ensure compliance with agency policies and procedures
- Develop and implement policies, procedures, and clinical guidelines
- Oversee staff scheduling, supervision, and ongoing education for clinical team members
- Help implement and monitor quality assurance programs
- Oversee maintenance of medical records to ensure accuracy and regulatory compliance
- Assist with staff training and educational programs
- Support a positive and collaborative team environment

Requirements:
- Minimum 5 years of experience as a Registered Nurse (RN)
- Minimum 2 years of supervisory experience in any healthcare setting
- Active RN license in the state
- Must be a U.S. Citizen or Green Card holder — no exceptions
- Hospice or home health experience is a plus
