Hospice Patient Care Manager / Clinical Manager (Registered Nurse)

We are hiring a Full-Time Registered Nurse (RN) for a Patient Care Manager (PCM) position in the Hospice Home Care setting.

This is a leadership opportunity for an experienced RN who enjoys mentoring staff, supporting quality patient care, and helping lead a compassionate hospice team.

What You’ll Do:

• Oversee delivery of patient services and ensure compliance with agency policies and procedures
• Develop and implement policies, procedures, and clinical guidelines
• Oversee staff scheduling, supervision, and ongoing education for clinical team members
• Help implement and monitor quality assurance programs
• Oversee maintenance of medical records to ensure accuracy and regulatory compliance
• Assist with staff training and educational programs
• Support a positive and collaborative team environment

Requirements:

• Minimum 5 years of experience as a Registered Nurse (RN)
• Minimum 2 years of supervisory experience in any healthcare setting
• Active RN license in the state
• Must be a U.S. Citizen or Green Card holder — no exceptions
• Hospice or home health experience is a plus
