Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
• Must be knowledgeable of quality assessment and assurance procedures

HOURS: (3) 12-hour Shifts 10am-10:30pm Monday-Friday with Two Weekend Shifts Per Month One Saturday and One Sunday (shift: 10am-10:30pm) Plus One Hot and One Cold Holiday required.

TERRITORY: South Chicago (Beverly and surrounding areas), IL.

What You�ll Do:

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
