Hospice - RN

Pennsylvania | Full-Time

Our client is seeking a Hospice RN to join a dedicated, supportive care team focused on delivering comfort, dignity, and quality of life to patients and their families.

What You’ll Do

• Manage and deliver individualized hospice care plans
• Provide compassionate, patient-centered care in home settings
• Support patients and families through education and guidance
• Complete timely, accurate documentation using EMR systems
• Coordinate with an interdisciplinary team to ensure continuity of care

What We're Looking For

• Active RN license in Pennsylvania
• Associate’s degree or higher in Nursing
• 2+ years of RN experience (hospice experience strongly preferred)
• Experience with EMR systems (TruChart or similar)
• Strong time management and ability to work independently
• Valid driver’s license, reliable transportation, and auto insurance

Schedule
Monday–Friday, 8:00am–5:00pm
On-call rotation: one weekend every 5–7 weeks (Fri–Sun, 8pm–8am)
Two holidays per year
