Are you a compassionate and skilled Registered Nurse who finds purpose in delivering meaningful, end-of-life care? Join our team as a Hospice Field RN, where your expertise and empathy directly impact patients and families during life’s most important moments.

In this role, you’ll provide patient-centered, holistic care to individuals facing life-limiting illnesses—supporting not just their medical needs, but their emotional and spiritual well-being. You’ll work independently in the field while collaborating closely with an interdisciplinary care team to ensure comfort, dignity, and quality of life.

What You’ll Do

• Perform comprehensive initial and ongoing patient assessments, addressing physical, functional, psychosocial, and environmental needs
• Develop, implement, and adjust individualized care plans to ensure continuity and quality of care
• Determine hospice eligibility and level of care through admission and recertification criteria
• Educate patients and families on disease progression, symptom management, and end-of-life care
• Coordinate care with physicians, social workers, chaplains, and the interdisciplinary team
• Provide clinical direction and oversight to Hospice Aides and LPNs/LVNs
• Document care thoroughly to reflect patient progress and outcomes
• Respond to changing patient needs and communicate updates to the care team
• Participate in on-call rotation to support patients and families when they need it most

What You Bring

• Active RN license
• CPR certification
• Valid driver’s license and auto insurance
• Minimum 2 years of RN experience (hospice experience preferred)
• Experience with HomeCare HomeBase is a plus
• Strong clinical judgment, communication skills, and the ability to work independently
