What You’ll Do

As a Hospice RN, you will provide compassionate, patient-centered care in the home or care facility setting while coordinating with an experienced hospice team.

Your responsibilities include:

• Performing comprehensive nursing assessments
• Developing and managing individualized care plans
• Providing symptom management and comfort-focused care
• Educating patients and families on disease progression and end-of-life care
• Collaborating with physicians, social workers, chaplains, aides, and the interdisciplinary team
• Supervising Hospice Aides and LPN/LVNs
• Monitoring patient condition and communicating changes promptly
• Completing accurate and timely documentation
• Supporting families emotionally through every stage of care
• Participating in on-call rotation as assigned

Qualifications

✔ Current RN License
✔ CPR Certification
✔ Valid Driver’s License & Auto Insurance
✔ Hospice RN experience preferred
✔ HomeCare HomeBase experience preferred
✔ Strong clinical, communication, and critical thinking skills
