A Typical Day for a Registered Nurse Case Manager with WesleyLife At Home Hospice:
Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall wellâbeing
Manage pain and other symptoms using evidenceâbased hospice practices
Coordinate medications, treatments, and supplies to support patient needs
Prepare and update individualized care plans in collaboration with the interdisciplinary team
Provide education on what to expect throughout the hospice journey
Teach family caregivers how to provide safe, comfortâfocused care
Offer a listening ear and calm presence during moments of uncertainty
