Responsibilities

As a Hospice Clinical Manager, you will:

  • Direct and coordinate hospice clinical services to ensure continuity, quality, and safety of patient care in compliance with state and federal regulations, including Conditions of Participation (CoPs)
  • Promote the philosophy, mission, and policies of the organization to support outstanding patient outcomes
  • Supervise and provide leadership to clinical staff, ensuring quality care delivery, timely assessments, reassessments, and continuity of care
  • Participate in hiring, training, staff development, and clinical oversight activities
  • Collaborate with the Interdisciplinary Group (IDG) in developing and reviewing individualized patient care plans
  • Monitor and lead QAPI (Quality Assurance and Performance Improvement) initiatives and implement corrective actions when needed
  • Ensure compliance with Medicare, Medicaid, and third-party payer requirements through education and coaching
  • Identify operational or clinical concerns and communicate recommendations to leadership through verbal and written reporting
  • Support infection prevention practices and promote adherence to standard precautions
  • Maintain effective communication and positive working relationships with staff, patients, families, physicians, and community partners
  • Participate in ongoing professional development and mandatory continuing education

Qualifications

Required:

  • Current and active RN license
  • Valid driver’s license and auto insurance
  • Current CPR certification
  • Strong leadership, communication, and organizational skills
  • Ability to manage multiple priorities in a fast-paced hospice environment

Preferred:

  • Minimum of 2 years of hospice management experience
  • HomeCare HomeBase experience
  • Experience leading interdisciplinary clinical teams
