Job Title: Hospice RN

Department: Mozaic Home Care and Hospice

Supervisor: Hospice Clinical Director

Job Objective:

The Hospice RN ensures quality and safe delivery of Hospice nursing services. This role encompasses On Call remote phone triage and visits in the field, both scheduled and as necessary. On Call services are rendered during nontraditional work hours providing service during weekend, evening, and holiday hours.

Minimum Qualifications:

Education: Graduate of an accredited school of nursing.

Experience: At least 2 years recent experience in the delivery of hospice nursing services

Licensure: Valid license as a Registered Nurse in the State of Connecticut

Essential Job Functions:

  1. Triage calls and speaks with patients and families to meet need i.e. medication adjustments, supply need, implementation of changes of POC.
  2. Provides assessment of patient care needs when necessary, during nontraditional work hours.
  3. Make home visits to meet any acute care needs to assess and intervene for changes in patients status
  4. Implements the coordinated plan of care of adult and geriatric clients served by the hospice and palliative care teams.
  5. Visits may happen during overnight hours, on weekends or holidays.
  6. RN Pronouncement at time of death including notification of funeral home, hospice team, attending MD and additional family members as needed.
  7. Assess patient and family needs, being aware of physical, emotional and spiritual aspects and gather data on social, economic and cultural factors which may influence health, well-being and quality of life.
  8. Perform Initial Assessment visits and Start of Care including obtaining required documentation for services, medication and DME and developing plan of care.
  9. Evaluate and perform ongoing assessment and revise plan of care with interdisciplinary collaboration as needs and condition of patient/family change.
  10. Attend and participate in care conferences.
  11. Perform other duties as assigned consistent with skills and training and the mission and goals of the agency

Physical and Mental Requirements:

  1. Ability to sit or stand for long periods of time.
  2. Ability to speak clearly and be understood.
  3. Able to hear sounds within some background of noise as in answering the phone, intercom, etc.
  4. Visual acuity to see computer and read printed documents.
  5. Ability to use company software and equipment to accomplish electronic tasks.
  6. Ability to concentrate on fine details with interruptions.
  7. Ability to attend to tasks/functions for more than 60 minutes at a time.
  8. Ability to understand and relate to the reasons behind several related concepts.
  9. Ability to remember multiple tasks /assignments given to self and others over long periods of time.
  10. Ability to perform gross body coordination such as walking, stooping, filing, etc.
  11. Ability to perform tasks which require hand eye coordination such as keyboard skills, writing or picking up paper.

Environmental Conditions:

  1. Rare exposure to blood, body tissue or fluids.
  2. Possible exposure to high/low temperatures.
