What You’ll Do:
- Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.
- Develop individualized plans of care.
- Complete face to face assessments and prescribe medications or treatments.
- Collaborate with the interdisciplinary team.
What You Bring:
- Active NP certification (state-licensed).
- Valid driver’s license, reliable transportation, and current auto insurance.
- Strong communication skills and a team-oriented mindset
- Ability to work independently in home settings
- Commitment to patient-focused care
- CPR Certification

Salary: $108,000 - $125,000 annually
