Our client is a company that supports patients suffering from Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by forming deep -rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. They are expanding and looking for experienced Nurse Practitioners to join their team to deliver personalized and compassionate medical care to our patients.

What You'll Do:<\/b>
<\/p>

  • This role will be a traveling position where you'll travel within a two -hour travel radius from your main location where they will be visiting the patient's in -home (80%+ road travel)
    <\/li>
  • Patient health assessments, conducting physicals, prescribing medications, maintaining accurate records, scheduling appointments, and follow -up appointments
    <\/li>
  • Work collaboratively with the care management team to ensure the patient receives quality, evidence -based care.

    Requirements<\/h3>

    What You'll Need:<\/b>
    <\/p>

    • This position requires that you have 2+ years of experience in any (1) of the following:
      <\/li>
    • Home Health Care
      <\/li>
    • Hospice
      <\/li>
    • Managed care
      <\/li>
    • Active and unrestricted Registered Nurse and Nurse Practitioner license in your state
      <\/li>
    • Board certified by ANCC or AANP Current and unrestricted DEA certificate
      <\/li>
    • Must be proficient with medical instruments and equipment required by the work
      <\/li>
    • Sound understanding of all federal and state regulations, including HIPAA and OSHA
      <\/li>
    • Experience in kidney disease control of related co -morbidities (HTN Diabetes CHF) preferred
      <\/li><\/ul>
