Palliative Care Clinic

Full-time day 40 hours

Mon-Fri 9-5, no weekends or holidays

We are seeking a highly organized and compassionate professional who can efficiently manage incoming calls, actively listen to patient needs, and quickly determine the most appropriate course of action. This individual will handle a high volume of daily interactions, ensuring each patient receives timely, accurate, and courteous assistance.

The ideal candidate is detail-oriented and proactive, capable of managing voice messages and in-basket communications with precision. They will prioritize responsiveness while maintaining a pleasant and professional demeanor, ensuring every patient interaction is positive, helpful, and handled with care.

·         The Palliative care team assists hospitalized patients who are experiencing a chronic debilitating or life limiting illness.

·         Palliative Nurse will provide holistic care for each individual: physical, psychosocial and spiritual. 

·         The goal of palliative care is to prevent and relieve suffering and to support the best possible quality of life for patients and their families regardless of the stage of the disease or the need for other therapies. 

·         Triage incoming phone calls and electronic communication; respond or escalate per protocols.

·         Gather requirements from internal and external customers and provide information via phone, electronic messaging and in person.

·         Conduct outbound calls or emails to communicate test results and/or directions from the provider.

·         Initiate and follow through on prior authorizations, referrals, and medication refills

·         Document results and other patient-related information in the electronic health record.

·         Support office staff with patient care, when coverage is needed, including rooming patients, taking vitals, obtaining a comprehensive medical history, administering vaccines, injections and medications, phlebotomy, point of care testing, and medication reconciliation.


QUALIFICATIONS:
For LPN:
Required Associate's Degree Nursing/Patient Care

1.      Required Certificate Nursing/Patient Care

2.      2 years Nursing/Patient Care Preferred

3.      Basic Life Support Program (BLS) - American Heart Association

4.      Licensed Practical Nurse (LPN) - State Licensing Board

For: Medical Assistant:

1. Required High School Diploma/GED

2. Required Program Medical Assisting Required Program In lieu of program, 5 years of medical assistant experience

3. Basic Life Support Program (BLS) - American Heart Association

Registered Medical Assistant (RMA) - American Allied Health

Certified Medical Assistant (CMA) - American Association of Medical Assistants (AAMA)

Medical Assistant (RMA) - American Medical Technologists (AMT)

Registered Medical Assistant (RMA) - American Registry of Medical Assistants

Nationally Registered Certified Medical Assistant (NRCMA) - National Association for Health Professionals (NAHP)

National Certified Medical Assistant (NCMA) - National Center for Competency Testing (NCCT)

Certified Clinical Medical Assistant (CCMA) - National Healthcareer Association (NHA)
