To provide spiritual and emotional care to chronically and terminally ill patients using the Palliative Care Transdisciplinary Model. The chaplain gathers and evaluates data pertinent to the patient’s situation including psycho-social-spiritual/religious health in collaboration with the transdisciplinary team of doctors, nurses, social workers, pharmacists, patient, families, and care givers. The chaplain develops and implements a plan of care to promote patient well-being, continuity of care, quality of life and end of life goals. The chaplain models and collaborates within the organization and its disciplinary team to provide competent patient and family centered care respecting the cultural diversity of the population we serve. The chaplain facilitates the completion of advance directives (living will and power of attorney for health care). The chaplain provides information to patients and families regarding community resources.
Provide pastoral care to referred patients/families and to staff/physicians.
Collaborate with palliative care team including:
Assist other bereavement support resources and spiritual care providers, i.e., community religious and spiritual care ministers, community referred agencies, et cetera, in maintaining high level of continuity of care.
KNOWLEDGE AND SKILLS:

EDUCATION: Bachelor’s required, masters preferred, four clinical pastoral education units, palliative care continuing education.
YEARS OF EXPERIENCE: Five years of experience in health care chaplaincy preferred.
REQUIRED SKILLS AND KNOWLEDGE: Clinical pastoral skills with emphasis in palliative and bereavement care.
