What You’ll Do:
- Completes an initial, comprehensive and ongoing assessments of patient and family to determine home health or hospice needs.
- Provide respectful, compassionate care in patients’ homes.
- Develop patient focused care plans that establish goals based on nursing diagnoses, and incorporate rehabilitative nursing actions.
- Collaborate with interdisciplinary team members.
What You Bring:
- Active nursing certification (state-licensed).
- Valid driver’s license, reliable transportation, and current auto insurance.
- Strong communication skills and a team-oriented mindset
- Ability to work independently in home settings
- Commitment to patient-focused care
- Current CPR Certification
