Registered Nurse (RN) - Hospice

**Title** Registered Nurse – Hospice **Schedule** 8am-4:30pm, Monday to Friday **Assignment Details** Contract Length: 13 weeks Guaranteed Hours: 72 hours biweekly **Requirements** - Must hold a valid state nursing license at the time of submission - BLS certification required **Roles & Responsibilities** - Provide nursing assessments and perform routine visits to patients in home and facility settings. - Manage a designated caseload of patients, conducting 4-5 visits per day. - Coordinate care with interdisciplinary teams to ensure comprehensive patient management. - Educate patients and families regarding care plans and available resources. - Document patient conditions and care provided accurately in electronic medical records. **Compliance / Notes** - A coversheet is required at the time of submission. - All requests for time off must be submitted at the time of application. - Please provide availability for interviews upon submission. - Mileage will be reimbursed according to IRS guidelines. - No local travelers will be accepted.
