Registered Nurse (RN) - Hospice

**Title:** Outpatient Hospice Admission RN – Hospice **Schedule:** - Shift: 10-hour days, 08:00–18:30 **Assignment Details:** - Contract Length: 13 weeks - Guaranteed Hours: 40 hours per week **Requirements:** - Current RN licensure in the appropriate state - BLS certification - Minimum 3 years of experience in home care or hospice - Familiarity with hospice admission LCD criteria (preferred) - Experience with Epic Dorothy/Comfort (preferred) **Roles & Responsibilities:** - Complete initial clinical hospice assessments and review patient history - Initiate plan of care and present summary to the Hospice Medical Director - Obtain necessary supplies, DME, and medications for new patients - Facilitate discharge planning for hospitalized patients - Educate patients and families about hospice services and support **Compliance / Notes:** - Orientation: Approximately 80 hours - RTO after booking is not allowed; no RTO during the first week - Must work 2 out of 3 major holidays - Flu vaccine required (medical/religious exemptions only)
