This position includes a $5,000 sign on bonus that will be paid out over the course of the first year. 


This role covers mostly the West Metro area and would be based out of St. Louis Park, MN.

SUMMARY:

The Registered Nurse (RN), Hospice provides end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setting.
 
JOB SPECIFICATIONS:

Education, Experience and Credentials

  • Graduate of an accredited school of nursing required.
  • Must possess and maintain State of Minnesota RN licensure.
  • One year of Clinical Nursing experience preferred.
  • Med-Surg, TCU, LTC, Assisted Living, Home Health, or Hospice experience preferred.
  • Possess a valid driver’s license with good driving record, and proof of valid insurance. Must have reliable transportation.
  • CPR Certification required.

Knowledge, Skills and Abilities

  • Excellent interpersonal, collaboration, and relationship building skills to effectively work with a diverse group/variety of people and personalities, including developing relationships with vendors, family members, volunteers, clients and staff.
  • Able to provide client care planning and client teaching skills.
  • Able to plan, prioritize, coordinate, and manage own work. Able to work unsupervised, make decisions and solve routine problems independently, effectively and creatively.
  • Work with a proactive, ownership approach to work and operate with a sense of urgency.
  • Understand and maintain confidential nature of organization and client information, including and in accordance with HIPAA regulations.
  • Able to assist residents with mobility needs such as transfers, ambulation, escorts or use of assistive devices. Follow established safety protocol and use proper body mechanics.
  • Work as an effective and proactive team-player; understands the importance of supporting the organization, clients, customers and other Sholom employees.
  • Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
  • Self-motivated, fast learner shows initiative, willing to seek out client and staff needs and meet them.
  • Communicate clearly and effectively both orally and in writing to all levels of people inside and outside the organization, including possessing good English-speaking skills, fluency and understandability.
  • Able to develop and give oral presentations to large and small groups of individuals inside and outside the organization.
  • Strong computer skills, including Microsoft Word, Excel, Outlook and other clinical applications.
  • Able to follow Sholom safety guidelines in all duties and responsibilities. 
  • Must be able to perform the duties of unlicensed staff.
  • Follow and adhere to Universal Precautions and Infection Control procedures.

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

Incumbent must be able to:

  • Perform heavy work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 75 pounds.
  • Push/pull clients in wheelchairs up to 200 pounds a distance of several feet.
  • Vision acuity to read fine print, warning labels, doctor prescriptions, etc.
  • Communicate clearly and effectively both orally and in writing, including possessing good English-speaking skills, fluency and understandability. 
  • Must be able to successfully pass training and education modules, and exhibit competency with medication administration and documentation.
  • Twist, turn, bend, stoop, kneel, squat and reach frequently in the performance of client assistance responsibilities.
  • Frequently stand and walk during an eight-hour shift.
  • Perform repetitive movements of the upper extremities.
  • Exert moderate physical energy for up to eight hours per day.
  • Apply manual dexterity for using and maintaining tools and equipment.
  • Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.

TOOLS AND EQUIPMENT USED

Incumbent must be able to use communication devices, medical equipment, client assistive equipment, walkers, wheelchairs, electric wheelchairs, telephone, calculator, copy machines, computers and medical equipment such as thermometers, blood pressure equipment, oxygen tanks, etc.

Compensation: An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. Shift and/or other differentials may be available to increase your pay rate for certain shifts or work.
