Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide Continuous Care overnight visits for patients and families in Palm Beach County. This role is ideal for an RN who thrives in one-on-one patient care and finds purpose in delivering comfort, dignity, and peace during life’s most vulnerable moments. Your shift would be: 7pm - 7:30am and every other weekend.

What you'll Do
As a Continuous Care RN, you’ll play a vital role in supporting patients and families through high-acuity hospice needs by providing attentive, skilled, and compassionate care.
- Support and uphold Trustbridge Hospice’s mission, vision, and values in every interaction
- Coordinate patient and family care under the direction of the primary physician and Registered Nurse
- Assess and identify patient and family needs, responding with timely interventions
- Collaborate as part of the Interdisciplinary Group (IDG) to follow and contribute to individualized plans of care
- Utilize skilled nursing practices—monitoring, interventions, and education—to enhance comfort and quality of life
- Provide compassionate crisis intervention and emotional support during critical moments
- Deliver personalized, sincere, and engaging care that prioritizes patient and family comfort above all else
- Ensure patient and family satisfaction by promptly addressing concerns with empathy and professionalism
- Maintain strict confidentiality and uphold the highest ethical standards in patient care
What You'll Need

Registered Nurse (RN) license, active and in good standing in the State of Florida
- BSN preferred
- Hospice certification preferred
- Current Basic Life Support (BLS) certification
- At least one year of experience in one or more of the following preferred:
- Hospice Nursing
- Home Health or Public Health Nursing
- Oncology Nursing
- Medical/Surgical Nursing
- Geriatric Nursing
- Strong understanding of Hospice Nursing principles, including:
- Case management
- Crisis intervention
- Communication and teaching skills
- Thanatology and end-of-life care needs
- Basic computer proficiency; EMR experience a plus
- Reliable transportation, valid Florida driver’s license, and required auto insurance
