Registered Nurse (RN) full time opening in our hospice division for patients residing in their own home or facilities/nursing homes we serve for Home Health & Hospice Care. Service Areas:  Patient homes in Southern New Hampshire greater Merrimack, Nashua, Manchester communities.

Responsibilities

• Completes an initial home health assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Utilizes the nursing process in planning, providing and evaluating skilled needs.
• Completes a comprehensive patient assessment.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.

Qualifications

• Graduate of an accredited school of nursing.
• Associate degree in nursing.
• Registered as RN with New Hampshire’s Board of Nursing.
• A minimum of one year recent acute care experience.
• Ability to exercise initiative and independent judgment.
• EPIC experience preferred, willing to train! 

Preferred Qualifications

• Bachelor degree in nursing.
• Community Health/Education experience.
• Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
• EMR software proficiency.

• Registered as a RN with New Hampshire’s Board of Nursing.
• CPR Certification.
• Satisfactory criminal record.
• Satisfactory motor vehicle record.
• Once fully trained minimum requirements is one weekend a month pays additional $4.00 an hour and 1 winter and 1 summer agency holidays required pays time and a half.
