Social Worker | Home Health & Hospice

Make a difference in the lives of others by working as a Social Worker with CentraCare’s Home Health and Hospice team!  As a vital member of our interdisciplinary team, the Social Worker plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards.

As part of our Home Health and Hospice team, you will have the opportunity to make a meaningful difference in the lives of those we serve while working in a collaborative and supportive environment.  This position will primarily serve patients in St. Cloud, Sartell, Sauk Rapids, Foley and surrounding areas.  

Schedule:

• Full-time | 72 hours every two weeks 
• Monday - Friday | 8:00 am – 5:00 pm 
• No weekends or holidays!
   

Pay and Benefits:

• Pay based on experience and degree
  • Bachelor's in Social Work starting at $30.65 per hour
  • Master's in Social Work starting at $33.15 per hour  
    • Pay range: $30.65 - $49.73 per hour

Qualifications:

• Bachelor's or Master’s degree in Social Work 
• Current Licensed Social Worker (LSW) in Minnesota 
• Valid Minnesota driver’s license, a driving record in good standing, reliable transportation and insurance liability coverage following CentraCare’s policy
   

To be eligible for the hiring bonus and relocation assistance, you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance also requires that you must currently reside more than 50 miles away from the Marketplace office in Waite Park, MN and relocate to within 30 miles of it within 120 days of employment.
