As a vital member of our interdisciplinary team, the Social Worker plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards.

To be eligible for the hiring bonus and relocation assistance, you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance also requires that you must currently reside more than 50 miles away from the Marketplace office in Waite Park, MN and relocate to within 30 miles of it within 120 days of employment.
