Carewell Supportive Care, a member of Empath Health is seeking a Palliative Care APRN. The APRN in the Community-Based Palliative Care Program provides comprehensive, patient‑centered care in collaboration with the Medical Director and interdisciplinary team. This role focuses on assessing complex needs, managing symptoms, and supporting patients and families through serious illness. The APRN develops and evaluates individualized care plans, promotes self‑care, and ensures alignment with patient and family goals. Through clinical expertise and compassionate communication, the APRN helps deliver high‑quality, holistic palliative care across hospital, community, and office settings.

What you'll Do:

  • APRN experienced in palliative care will provide services Monday-Saturday 8am-5pm which includes a rotating Saturday schedule
  • The APRN will provide consults to 3 area hospitals in the Ocala area
  • Our patient community will be a mix of hospital and office based visits
  • This APRN will be a full-time team member service our community needs
  • The Palliative care APRN team member will provide symptom management, advanced care planning and goals of care
  • Our APRN palliative care diagnoses, treats, and monitors acute and chronic diseases in collaboration with a physician.

What You'll Need:

  • Education: Master’s degree in nursing or a related field.
  • Board Certification
  • Licensure: Current Florida license as an Advanced Registered Nurse Practitioner, certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.
  • Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
  • NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements).
  • Minimum of two (2) years of nursing experience is required.
  • Minimum of two (2) year experience as an APRN with experience in adult, gerontology, or oncology practice preferred.
