About the Role:
The Hospice Home Team RN Case Manager plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses wherever they call home. This is a full-time, salaried position working a flexible Monday - Friday 8-4:30pm schedule with a minimal on-call requirement. The primarily territory for this role is Sheboygan County, but we also serve patients in Manitowoc & Ozaukee Counties.
This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The RN Case Manager collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to ensure seamless communication and continuity of care. By conducting thorough assessments and monitoring patient progress, the RN Case Manager ensures that care is both effective and aligned with the patient’s goals and wishes. Ultimately, this role aims to enhance the quality of life for patients while supporting their families through the hospice journey with dignity and respect.
