DescriptionThis role will provide social work support across our South territory, with a primary focus on Pauls Valley, Lindsay, and the southern part of Norman. Regular travel within this region is expected.
Duties/Responsibilities:
- Participates in referral evaluation to ensure appropriate admissions.
- Implements hospice information visits and signs up visits as directed.
- Assesses patients and their families for psychological needs and concerns according to policy.
- Participates in developing the full Interdisciplinary Team plan of care according to policy.
- Coordinates psychological care to meet needs and concerns of the patient and family in a manner that is meaningful to them.
- Acts as a liaison as needed between hospice team and patient/family support systems.
- Visits hospice patients/families where directed by the IDT.
- Documents all visits and phone calls, and places documentation in clinical records according to policy.
- Serves as a member of IDT and attends team meetings.
- Utilizes all available resources such as community agencies to assist patients and their families so they may live better within the limitations of their illness.
- Responds to patient/family needs promptly after hours when serving on call.
- Participates in continuing education for hospice staff.
- Coordinates or participates in presenting hospice programs for staff, volunteers and the communities as large.
- Any and all other duties and responsibilities as assigned.

Education, Licenses, Certifications and Experience:
- Masters degree in social work.
- 1-2 years experience in home health or hospice, LTCF preferred.

- Communication: ability to read, speak, and hear in English.
- Strength: sedentary position (exerting up to 10lbs of force occasionally)
