Part Time

Direct report to Regional Executive Director

Works with the Marketing, Admissions Department and Branch Directors

Manages the process and performs visits as needed

 

 

Responsible for:

Managing the overall process and development of the Palliative program

With a strong growth trajectory of 20-30% Hospice growth from transitioning from palliative to hospice when medically eligible and in alignment with the patients goals of care. 

 

Duties and Responsibilities

Goals of Care discussion with patients and families

Strong Compliant Growth initiative

Perform Palliative visits and initiate symptom management interventions

Coach and Train Palliative staff to hit growth targets

Mindful of budgets and census status

Perform visits as needed

Strategic Account/Partner development with Compassion Care

Strong initiative to make sure front-end billing is accurately recorded by the admissions coordinator.

Personally, and professionally responsible for growth

Responsible to managing the Palliative care platform

Be personally accountable for growth and cultural standards

New staff field recruitment efforts to support growth

Perform visit with compassion and understanding with no judgement

Strong communication skills and teamwork skills with patients, families, hospice, leadership and Liaisons

 Serve as a resource for patients and families

 

Technical daily/weekly tasks

Responds to emails and calls promptly

Scheduled patient visits timely [within 48-72 hours of scheduling call]

Monthly report to be sent to Leadership timely

1 growth mtg in OKC per week

1 growth mtg in Tulsa per week

Maintains and follows through with weekly schedule

Completes required charting promptly within same day as visits

Reviews patient charts and MR  consistently

Attend Marketing stand up calls daily.

Work closely with the Admissions Coordinator and Regional Executive Director and Branch Directors on productively moving the RTA process in a timely manner to hospice.

Complete a seamless approach to transitional care for the patient and families
