Registered Nurse (RN) - Hospice

**Title** RN Case Manager **Schedule** M-F, 8-5. On-call and weekend rotation. **Assignment Details** Contract Length: 13 weeks Guaranteed Hours: 40 **Requirements** - Current RN license in the state is required (BSN preferred). - Valid driver’s license, appropriate auto insurance, and reliable transportation required. - 2 years of recent acute care, home care, or hospice experience preferred. - Clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred. - Experience with HCHB is strongly preferred. **Roles & Responsibilities** - Provide skilled nursing and coordinate care for patients and their families in a residential setting. - Observe, assess, evaluate, and plan interventions based on patients’ physical and psychosocial statuses. - Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care. - Assure continuity of care through effective communication and coordination with other disciplines and support agencies. - Maintain accurate and complete patient records in accordance with agency standards. - Participate in team scheduling and on-call rotation to meet patient needs. - Engage in quality improvement activities and attend training as assigned.
