Registered Nurse (RN) - Hospice

**Title** Hospice RN – Home Health / Hospice **Schedule** Shift: 7 on / 7 off; 10-hour shifts — Fri–Thurs, 12:00pm–10:30pm **Requirements** - Minimum: 1 year of skilled hospice experience - Home Care/Home Health experience required - HCHB charting experience required - Required certification: BLS **Roles & Responsibilities** - Conduct patient admissions, triage, and RN visits in the home health and hospice settings. - Provide symptom and pain assessment, medication management, and patient/family education and support. - Offer care and support related to death and dying, including pronouncement. - Document patient care accurately using HCHB and EPIC charting systems. **Compliance / Notes** - Transportation: Provider expected to have reliable personal transportation. - Modules/orientation: Pre-start modules required (internal tracking); time spent is self-reported.
