RN Case Manager, Hospice [Per-Diem On Call PM's]

We are hiring a Hospice RN to help cover our On-Call overnights during the week; Monday - Wednesday from 5 pm to 8 am.  

On Call Visit Rates:
$195 base plus $11 gas stipend = $206 per/Evaluation
$105 base plus $11 gas stipend = $116 per/routine

What you will do:

• Have flexible hours and working close to home (reduce your commute!)
• Complete initial and on-going comprehensive assessments of the patient’s needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs

• Graduate of an approved school of professional nursing.
• Current, active license in the state of  California Board of Nursing, 
• Current CPR, health clearance, TB clearance.
