Registered Nurse - Home Health/Hospice

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 - 17:00). On-call rotation required during weekends.

Job Description

The Registered Nurse will provide care in a home health and hospice setting, focusing on end-of-life care for patients with serious illnesses. The role requires a compassionate approach to patient care, ensuring that families receive support during a difficult time. Responsibilities also include medication administration and pain management, along with symptom assessment and management.

Responsibilities

• Conduct patient intake assessments and interviews.
• Administer medications and manage pain using pharmacologic and non-pharmacologic interventions.
• Perform symptom assessments and manage complications such as altered mental status and nausea.
• Provide education and support to patients and their families regarding treatment options and care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Participate in on-call rotations for urgent patient visits as needed.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience required.
• Active RN license in the state required.
• BLS certification is required.
• Previous experience in home health or hospice preferred.
• Epic experience is preferred.
• End-of-Life care experience is highly desirable.
