Trustbridge Hospice, a proud member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Admissions Team serving Central Palm Beach County. This is a meaningful opportunity for an RN who is passionate about guiding patients and families through one of life’s most significant transitions with empathy, clarity, and dignity. You will work the following schedule: Monday/Tuesday/Thursday/Friday 9:00am - 7:30pm.

What you'll Do
As an Admissions Nurse, you will play a vital role as both a clinical expert and a trusted guide for patients, families, and healthcare partners:
- Serve as the primary nurse responsible for evaluating patients referred for hospice services
- Educate patients and families on hospice care and clearly communicate all components of our continuum of care
- Collaborate with referring physicians and liaisons to coordinate individualized, hospiceâappropriate plans of care
- Act as a key liaison between Trustbridge Hospice and hospitals, physicians, longâterm care facilities, and other healthcare professionals
- Promote and advocate for the hospice philosophy through education and relationshipâbuilding within the healthcare community

Your work ensures patients receive timely, compassionate care while helping families feel informed, supported, and confident in their hospice journey.
What You'll Need
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred
- Hospice certification preferred.
- Current Basic Life Support Certification.
- Possess the ability to assess physical and psychosocial needs of patient and family.
- Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
