Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the northern Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you'll Do

As a Hospice RN Case Manager, you will play a central role in coordinating high-quality hospice care while supporting patients and families with empathy and respect.

  • Uphold and actively support the organization’s mission, vision, and values
  • Coordinate comprehensive care for patients and families in collaboration with the primary physician and interdisciplinary care team
  • Identify and assess patient and family needs, ensuring individualized and responsive care
  • Serve as an integral member of the Interdisciplinary Group (IDG), contributing to and following a personalized plan of care
  • Utilize strong clinical assessment, monitoring, intervention, and teaching skills to maximize patient comfort and quality of life
  • Provide compassionate guidance and education to patients and caregivers throughout the hospice journey
  • Prioritize comfort and care as the highest standard in all patient and family interactions
  • Deliver consistently personalized, sincere, and engaging service
  • Ensure patient and family satisfaction by promptly and effectively addressing concerns
  • Maintain the highest level of confidentiality and respect in all aspects of patient and family care

 What You'll Need

  • Registered Nurse currently licensed in the State of Florida.
  • BSN preferred
  • Hospice certification preferred.
  • Current Basic Life Support Certification.
  • One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
  • Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
