Job Summary
The Hospice RN Case Manager provides comprehensive nursing care to hospice patients through assessment, planning, interventions, and evaluation. This role ensures high-quality, patient-centered care while serving as a liaison between the patient, family, and provider, supporting comfort and dignity at end of life.
Key Responsibilities
• Perform thorough assessments and reassessments of patient conditions.
• Coordinate effective patient and family education, discharge, and transfer planning.
• Identify and report signs of abuse per protocol.
• Provide culturally competent and developmentally appropriate care.
• Manage pain and symptoms using evidence-based techniques.
• Monitor and report changes in patient condition to physicians.
• Respond effectively in emergency situations.
• Rotate on-call responsibilities with nursing staff.
• Complete timely admissions and comply with agency policies.
• Implement nursing interventions based on physician orders.
• Maintain professionalism, confidentiality, and adherence to regulatory standards.
• Submit accurate and timely documentation.
• Support orientation of new employees when assigned.
• Demonstrate cost-effective use of supplies and resources.
• Promote high standards of clinical care without discrimination.
• Participate in agency and community hospice education programs.
• Oversee and coordinate all aspects of patient care for assigned caseload.
• Ensure understanding of hospice admission criteria and services.
• Demonstrate proficiency in palliative and end-of-life care.
• Update care plans according to changes in condition and regulations.
• Collaborate with the interdisciplinary team (IDT) and participate in meetings.
• Supervise home care aides and ensure required supervisory visits.
• Perform patient intakes and informational hospice visits as needed.
• Engage in performance improvement initiatives.
• Communicate care needs and updates to patients, families, and providers.
• Maintain continuity of care during transfers or referrals.
• Ensure complete and accurate clinical documentation.
• Maintain a caseload of approximately 18–20 patient visits per week.
• Perform additional duties as assigned.
Job Qualifications
Required:
• Graduate of an accredited school of nursing.
• Current RN license (Arkansas or Compact).
• One year of hospital or nursing home experience.
• Interest in geriatrics; experience preferred.
• AHA BLS certification.
• Reliable transportation and valid Arkansas driver’s license.
• Ability to assess, evaluate, and communicate effectively.
• Proficient in decision-making and patient care.
• English reading, writing, and speaking proficiency.
Preferred:
• Bachelor of Science in Nursing (BSN).
• Certified Hospice and Palliative Nurse (CHPN).
• Previous hospice experience.
