Midland Care Connection is a not for profit, community based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

Hospice House

A peaceful, 12 bed, in patient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team—nurses, social workers, chaplains, and other specialists—focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life.

Hospice House Care Team Coordinator

Helps oversee and organize the care provided to patients staying in the hospice house. The role involves working closely with nurses, doctors, social workers, chaplains, aides, and family members to ensure patients receive compassionate and coordinated end of life care. A coordinator typically manages communication between the care team, helps with admissions and scheduling, updates care plans, and ensures that patients’ physical, emotional, and spiritual needs are being addressed. They also provide support and guidance to families during a difficult time while helping maintain a calm, comfortable, and supportive environment within the hospice house. The position combines administrative coordination with strong interpersonal and emotional support skills, making teamwork, organization, empathy, and communication especially important in the role.



Qualifications

Required  

  • Strong communication and interpersonal skills 
  • Excellent organizational and time-management abilities 
  • Ability to work effectively with patients, families, and interdisciplinary care teams 
  • Compassionate, dependable, and emotionally resilient personality 
  • Basic computer and documentation skills 
  • Ability to handle sensitive end-of-life situations professionally 
  • Prior healthcare, customer service, or care coordination experience 
  • Ability to multitask in a fast-paced healthcare environment 
  • Valid driver’s license and successful background check  
  • Bachelor’s degree in nursing

Preferred  

  • Experience in hospice, palliative care, or healthcare settings
  • Experience working with older adults or families in crisis 
  • Background in healthcare administration, case management, or nonprofit healthcare 
  • Familiarity with electronic medical records (EMR) systems 
  • Knowledge of hospice regulations and patient-centered care practices 
  • Strong problem-solving and conflict-resolution skills 
  • Ability to provide emotional support and compassionate customer service 
  • Experience collaborating with interdisciplinary healthcare team
  • Flexibility and adaptability in emotionally demanding situation
